You may want to hire Zogby International to conduct such type of survey. Anyway here are my observations.
I grew up in a village located between Toba Tek Singh and Jhang. There were between 150-200 adults in the village. But only a handful, no more than 5-10, attended the ritual Namaz at the Mosque. All attendees were men. For Namaz Isha only Mulvi and two teens (including me) showed up. Attendance for Friday Namaz increased to about 20 men. Attendance tripled during Ramadan. But it was still a minority performing the ritual. Most farmers were busy with their work in the fields and could not participate in the five daily prayers. Also, most attendees were old folks. When village headman joined the Tableeghi Jamaat and become a regular Namazi, more people started performing the ritual Namaz. It was only Namaz Eid that almost all adults in the village attended. As far as I can recall only 25% folks observed fasting during the entire month of Ramadan. This percentage went up slightly during the last 10 days of Ramadan.
